Learn more about Vail Health <a href=\"https:\/\/www.vailhealth.org\/about\">here<\/a>.<\/p>\n<\/div>\n<div><strong>About the opportunity:<\/strong><\/div>\n<div>\u00a0<\/div>\n<div>The Nurse Navigator role is intended to guide the entire life-cycle of patient care within the department. The Navigator is the liaison between all components of care; providing information seamlessly to the patient\/ family, and between care providers in every care setting the patient uses.<\/div>\n<div>\u00a0<\/div>\n<div><strong>What you will do:<\/strong><\/div>\n<div>\n<ul>\n<li>Consult with patients by phone or in person to determine specific needs, determine referrals to care.<\/li>\n<li>Build a patient-family centered relationship.<\/li>\n<li>Provide initial admission assessment, screenings, and assess for barriers to care.<\/li>\n<li>Report to team.<\/li>\n<li>Attend and\/or review provider appointments; reinforce education and information provided by caregivers.<\/li>\n<li>Develop and deliver a Care Plan with treatment summary to patients and\/or sign consents and declinations and reinforce education on treatments accepted or declined.<\/li>\n<li>Organize evidence-based care plan and present to patients for discussion.<\/li>\n<li>Follow up as required with patient and providers.<\/li>\n<li>Sustain a patient centered relationship with the patient and family throughout the course of care and into post-care as needed.<\/li>\n<\/ul>\n<\/div>\n<div><strong>This description is not intended and should not be construed to be an exhaustive list of all responsibilities, skills and efforts or work conditions associated with the job.
